The Rockingham Community Care Program is the result of a significant grant that was awarded to Rockingham County from the State of New Hampshire and the New Hampshire Department of Health and Human Services. This generous funding underscores the commitment of both the state and our local community to support and improve the lives of older adults.

How it works

Participant Requirements

We assist older adults who:

  • Are residents of Rockingham County
  • Are 65 years of age or older.
  • Are dually eligible for Medicare and Medicaid*
Participants must also meet partner specific criteria to attend day programming. 
 
*Individuals who are Medicaid pending are encouraged to apply
